Insurance

We are a preferred provider with Aetna, Cigna, Delta Dental Premier, United Concordia Elite, and Met Life. Our pediatric dentist Dr. Cwiak and Dr. Alpano is now a Medicaid provider. Dr. McCormick and Dr. Nguyen are not Medicaid providers. We will file your claims once treatment has started.

Many insurance policies have a lifetime orthodontic benefit that is separate from regular dental insurance. Remember that insurance policies vary; we will review your insurance to determine the appropriate course of action.

To help us file your insurance, we require that all patients complete all patient forms.

If, during the course of treatment, your benefits change, the financially responsible party is accountable for all charges.
